Missoula Children's Theatre: The Jungle Book, August 4, 2025 10:00AM

For good and valuable consideration, the receipt of which is hereby acknowledged, I, the parent or guardian, hereby grant Mount Baker Theatre permission to use my child's likeness in a photograph in any and all of its publications, including but not limited to all of Mount Baker Theatre’s printed and digital publications. I understand and agree that any photograph using my child's likeness will become the property of Mount Baker Theatre and will not be returned. I acknowledge that since my child's participation with Mount Baker Theatre is voluntary, they will receive no financial compensation.I hereby irrevocably authorize Mount Baker Theatre to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing Mount Baker Theatre’s programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my child's likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph.I hereby hold harmless and release and forever discharge Mount Baker Theatre from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

The day camp is non-ticketed. Attendees will check-in at the door and must bring a copy of their e-mail confirmation. Emergency contact forms will be sent via email and must be filled out prior to the first day of the program.

Scheduled camp times begin and end promptly. Parents/guardians are responsible for transportation to and from the daily classes. Staff may ask parents/guardians for Photo ID to confirm they have permission to pick up child. A late fee will be administered if any child is not picked up on time each day.

Cancellations must be made at least two weeks prior to the program in order to be eligible for a full refund.
